I am not sure how best to ask this. but I have a coworker I am having issues with. I am really struggling. at first she wasn mad at me.. then she forgave and she keeps it so formal. I cant get an ounce of informality. our first conversation seems great. we shared Birthday dates and all that. I must have said something she didnt like on that conversation becauee after that conversation it went down hill.
I cant gt a number or even go on break. she says she prefer to chat Via the office IM.
I am so lost.

maybe you're just trying too hard, just talk to her normally and treat her like a friend. Have some friendly banter, be kind and she'll definitely become friends with you. And also as many people have already said let her know you're leaving
