At 4 years old I'd think this child needs a different outlet that
1. Supplies them with adequate attention and self worth
2. Wears them out so they don't want to feel pain any more.
But that's my sideways aunt outlook and I'm in no way a professional on the matter.

Things that should alarm more so are, a grown child at 6-9 messing in their pants, that is a known direct cause of sexual child abuse .
What you explained is different.
When does the 4 year old bite themself?
Do you notice a pattern of the times it occurs or any correlation to the behavior and their surroundings?

I thought this was your child and you were asking about her current behavior.
Perhaps her behavior was not caught in time.
I feel like this is some in depth case study stuff. Why do you ask? Who is she to you?
