Exactly as he said: he was buying meds in front of a patient and it obviously made him feel uncomfortable.
Maybe he is afraid it will change the way you see him?
Or maybe he didn't want to you know about that part of his private life because he likes to keep private and professional seperated.

I ran into my physiotherapist at the pharmacy this morning and saw that he was visibly uncomfortable to buy his medication in my presence. He even gave me a piercing look and mumbled something to the effect “now I’m buying medication in my patient’s presence” and then proceeded to order “the same like the last time” obviously concealing the name of the medication. Why was he so secretive? Even if I heard the medication name, I do not work in the medicine field, so I’d most probably do not know what his medication’s for. If the roles were reversed I’d have no problem to buy medication in his presence. I jokingly said that if that was a problem, he might have let me cut in front of him and buy my stuff first.
The relationship is purely professional and although the conversations are flowing when I have physiotherapy, it is only friendly bunter. I found out by accident his star sign, when he asked me about my hobbies and I mentioned one of them being astrology and asked what his star sign was.
Go figure. To be honest I felt slightly offended of his reaction.

I'd be uncomfortable too. Medication is a private thing.
How would you feel buying medication in front of your boss for example?
