Yes that is the sign of a family where the mother was badly hurt in some way, or grandmother. All the equivalents too, moon conj saturn, moon in cap, saturn in cancer, saturn in 4th...cancer MC.
Saddest placement...
If its considered good...a person would have nerves of steel and can't be easily hurt, I guess...because this person have been dealing with some darkness for a while so they become quite cynical about it.

Does this mean that one would have a difficult relationship with their mother?

Yeah, I don't think it would lead to some kind of fight with the mother for that Mars would need to be involved.
I think it would be a practical relationship, the mother would be practical and distant, the children could feel a lack of warmth from the mother.
The moon is the mother, Saturn normally make things real and practical " If you want it, you need to work for it ". With a square would be a tension between these two energies. A mother isnt suppose to treat you as a employee...this is what could happen with this placement IMO.
moon opposite saturn here. yeah, she is badly hurt and couldn't love me enough when I was young. Now, it is okay.

Insecurities and inability to succeed in achieving stuff. And when something is achieved, no one appreciates it.

Bad too but I think the moon is worse, the moon is more vulnerable and can lead to serious problems...Like depression and etc.
A blocked Sun must sucks indeed...

The moon represents one's emotions and the mother, Saturn in the teacher of zodiac and the father.
This aspect could show a bad relationship between the parents, having your emotional side being tested often through challenges or bad ships, feeling emotionally distant, lonely and the list goes on...
Oppositions can be harsh too
