I'm thinking it will be the same way it is with forced immunizations for school children. You will be able to get out of it with a letter stating that it is against your religious beliefs.

The Amish are forbidden by Ordnung law/rules to have any type of insurance.
If the healthcare is going to make every American citizen have health insurance .. then what happens to the Amish?
If they are to be pardoned, then every American who suddenly proclaims to be Amish will have to be excused, also, since there is no way to dispute a persons religious beliefs.
