As a man, what you've described is something I've never really done. But I think anyone could reasonably think of several reasons:
1. He has a rash.
2. He has an STD, like crabs.
3. He cut himself shaving.
4. He failed to shave and the stubble is irritating.
5. His underwear is too tight.
6. His underwear is too loose.
7. He forgot to wear underwear when he's not used to being commando, and the zipper is irritating him.
8. He recently had sex and didn't wash up well, now the skin is dry and itchy.
