My friend is a doctor and she says despite trying to be professional when they have to deal with taking/cutting off peoples clothes in the emergency room, her colleagues do indeed comment on the patient's underwear. Her advice was to always wear your best underwear and never your granny pants as you never know who is going to see them lol
