Muslims follow Muhammad, end of. You have to accept that Allah speaks through Muhammad, that every command was relayed through him. That's why you have proscribed prayers, halal requirements, essentially Jewish Law (although somewhat corrupted), specific required recitations, ablution, etc. Allah didn't tell anyone but Muhammad to do these things. So you follow Muhammad.

Next question: how did Muhammad know that the angel in the cave was Gabriel? Could've been a jinn. Only Khadija seemed to be sure.
