I don't want to waste my time if you aren't feeling anything anymore. I thought you'd be off my mind by now but you're not.
I've been trying to make it up to you for my hot and cold behaviour in the end, but at the same token I don't know if you are just unsure of the situation so coming off a little distant or if I'm just not getting the picture. I'm kind of getting mixed signals, and I'm sure that's probably how I made you feel at one point so I do apologise for that.
I'm a big girl and you know I'm pretty straight forward, so I can handle the truth and would respect you for your honesty if that's the case.
Don't get me wrong, I'm not expecting anything, just trying something different for once and vocalising my thoughts rather than disappearing. Take your time with your reply, but please just keep it real with me 🙂

I'm contemplating it.
I haven't liked someone in 3 years (dated people and had interests in them, but not felt this crazy connection with them like I do this guy).
we were dating a few months back and I became very distant and would disappear because I honestly just couldn't deal with how I felt for him, it scared me. Things became stagnant and we stopped seeing each other.
I saw him on the weekend for the first time in 3 months and I'm confused as to why he would want to catch up and let me crash at his house (didn't do anything sexual, just kissed and spooned) yet I feel he is being very distant with me now.
I just want some clarity on where I stand with him really but I don't want to come across as pushy.

you say you want him to keep it real but you've actually not asked him any questions but danced around your own behaviour and expectations.

Or something even shorter and more direct - just use what you wrote in your second sentence:

"I like you, you've been on my mind and I just want to know if the feelings mutual"

What sign is your mercury? Mine's Virgo, so I personally prefer something short and straight to the point. I have no patience for "dancey", flip floppy, abstract language.
